Intercare Physical Rehabilitation Hospital (Pty) Ltd in Newlands, Cape Town, seeks to recruit a Social Worker who will be responsible for providing emotional support, counselling, and resource coordination for patients recovering from injuries, surgeries, or chronic conditions. Acting as a liaison between patients, families, and the interdisciplinary team, ensuring clear communication, facilitating access to services, and promoting a positive patient experience for optimal recovery and reintegration into daily life.

CRITICAL OUTPUTS

  • Assess patients' psychosocial needs and provide emotional support.
  • Advocate for patient rights and coordinate resources for optimal recovery.
  • Act as a liaison between patients, families, and healthcare teams to ensure clear communication.
  • Develop individualized discharge plans, including home care and community resource coordination.
  • Provide counselling and education on coping strategies, healthcare options, and lifestyle adjustments.
  • Work collaboratively with interdisciplinary teams to implement holistic care plans.
  • Address urgent psychosocial concerns such as financial hardship, housing insecurity, and mental health crises.
  • Maintain accurate patient records and ensure compliance with healthcare regulations.

REQUIREMENTS

  • Education: Bachelor of Social Work (BSW) degree & SACSSP registration.
  • Experience: 3–5 years as a Social Worker, preferably in healthcare or physical rehabilitation.
  • Other:
    • Code 8 driver’s license and own reliable transport.
    • Ability to work in a pressurized environment with on-call and overtime requirements.
    • Strong interpersonal, communication, and ethical decision-making skills.
    • Experience in interdisciplinary collaboration.
